The closed-loop counterpulsation (CLCP) technique is a circulatory support technique in which the pump, placed around the left ventricle, injects and collects pulsatile flow to counter high intrathoracic pressure. The objective of this study was to develop a mathematical model of aortic stenosis (AS) with CLCP. The model was then used to evaluate the effects of systemic blood pressure (BP) on the hemodynamic efficiency of the CLCP technique. We performed a literature review, implemented the knowledge into a model, simulated the effect of various BPs, and compared the simulation results with our clinical experience. The model for AS with CLCP consisted of a pulsatile left ventricle with aortic stenosis. Aortic valve area (AVA) was 0.6cm2, compliance was 0.2mL/mmHg/mL, and time constant was 150ms. BPs of 100, 150, and 200mmHg were used for the simulations. The CLCP was effective at BP levels below about 200mmHg, but not below 100mmHg. When the CLCP was ineffective, the valve area was about 50% larger than that of a normal aortic valve. Our mathematical model of aortic stenosis with CLCP is useful for evaluating the hemodynamic efficiency of the technique.AP
